Fulfilling the legal duty pursuant to Article 13 of the Regulation of the European Parliament and the Council (EU) 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the European Union of 04.05.2016 L 119/1), hereinafter “RODO”, COMgraph Sp. z o.o. ul. Goleniowska 55c, 70-847 Szczecin NIP 851-10-18-057 hereby informs you that:

1) COMgraph Sp. z o.o. ul. Goleniowska 55c, 70-847 Szczecin NIP 851-10-18-057 is your Personal Data Controller, hereinafter the “PDC”;
2) Personal data is processed for the purpose of the due performance of an agreement (delivery/service agreement) by the PDC under Article 6, point 1, letter a), b), f);
3) The CDC’s legitimate interest is the need to perform its basic services related to the due performance of the agreement and direct marketing;
4) Your personal data shall not be transferred to a third country or an international organization. The data shall be delivered only to legally authorized institutions;
5) Your personal data shall be processed solely for the period necessary to fulfil the purposes of personal data processing arising out of the appropriate legal provisions;
6) You have the right to access your personal data, to rectify, remove it and to restrict or object to its processing;
7) You have the right to lodge a complaint with the President of the Data Protection Office (PUODO), ul. Stawki 2 00-193 Warszawa, tel. 22 531 03 00, fax. 22 531 03 01, e-mail: kancelaria@puodo.gov.pl;
8) The provision of personal data by you is voluntary but necessary for the purpose of the performance of this contractual relationship. Refusal to provide such data on your part shall necessitate the termination of this service agreement between PDC and you;
9) Your personal data shall not be subject to automatized decision-making processes by PDC, including profiling during the performance of this agreement.
